iRefer in Secondary Care

Hi,

Has anyone successfully integrated iRefer in Secondary care, especially integration with Oracle Cerner Millenium as the Ordering system? I am reaching out to confirm how other Oracle Cerner hospitals handle iRefer outputs, specifically:

1. Do they consume CDS/iRefer outputs when they arrive in OBX segments on the HL7 order messages (e.g., ORM/ORU), or do they expect them in different segments?

2. If they don’t accept OBX, which segment(s) do they recommend for these fields to ensure the CDS outcome, clarifications, and score are visible to reporters?

Our iRefer CDS-Cerner integration is set up so that the Orderwise fields for “Clinical history” and “Orderwise clarifications” are passed through as NTE segments and therefore appear in the Clinical History for the request in CRIS. The Orderwise score, additional information and tracking number are all passed through as OBX segments and therefore populate the “Event comment” section in CRIS.

This means that the name of the guideline and any information entered into CDS by the requester appears in the clinical history field, along with the clinical history they then type into the Cerner OEF afterwards, while things like EGFR/D-Dimer/Creatinine results, whether the request is “indicated” or not and the tracking ID all appear in the event comment section instead.

However, this mapping is specific to CRIS - your RIS may have different mappings for the details in Radiology requests.
