Sharing of static/mobile vans-DMWL

Dear all- re: the national mandate to bring MRI/CT vans under central control for another few months - has anyone been asked to share statics for PTL sharing and backlog clearance? We are being asked to share our static and carve out slots for other Trusts to direct book into- so pt’s from St Elsewhere to come to site- but obviously we don’t want to manual send IEP back to the other site with associated rubbish manual data entry. Does anyone have a router sorted for this to enable DMWL to follow the pt and send back to St Elsewhere? Have NHSx/d/i organised a central solution? with thanks- I hope this makes sense … we do not have a shared RIS or PACS…

We had a similar request but for a research scanner but our network team could not configure the use of the same IP for both local use and use by another Trust. Unfortunately at the time of the request we were not given much time to look into getting a router to split the sending of images and splittng of the patient DMWL on the same scanner to different Trusts. We ended up getting the modality engineer to come to site and change the configurations from the Other Trust details to the local details but became to much of a chnage to continue to use this. We ended up just importing the local exams to PACS which of course required manual PACS merged to the RIS record.

Things to look into:

  1. Check if the scanner can have different configuration profiles configured which will automatically apply different Modality IP, AET and PACS and DMWL configurations when selected. This way you could just swap to a different network port which is set for local setup and back to the original network port which is setup for a different Trust.
  2. Sectra has a new Sys router that they using for most new Trust deployments including ours so might be worth checking if this router can be used to fit your workflows.

This is something I have been trying to look at between Salford and Pennine Acute with sectra as we are sharing a ct and MR scanner and booking in blocks of 2 weeks.
I was hoping that as we shared a single central connectivity hub in Manchester as a back up for local connectivity hubs we would be able to utilise this To provide work-lists to the vans. It was also hoped as a result of a shared Pacs system we would be able to route data based on the accession number via a single connection into the correct silo within the shared archive, but due to deployment timescales for other trust and issues testing we have not achieved this as yet. Will update if we achieve this soon.

We use CAM CRIS to do this quite well with some help from the PACS supplier

DMWL work ok as the other trust using the network scanner books into our room Trust A by opening up that site in XR settings for certain key booking staff at trust B so the worklists appear without issue as the room is polling for work for itself it has all the identifiers of the other trust id and accession number

Then Philips built us a mini pacs archive with parsing rules we call it CT in a Box which look for the prefix of the accession number and routes that to the appropriate PACS, once firewall etc. have been allowed.

Trust B then reconciles the event in CRIS and assigns to for reporting at the originating site so reporting is done there scanning only done by us

We have this working between 4 sites at the minute and adding more in future
